There are forms that you will need to take with you to the doctor/dentist for these evaluations. The school has these forms available at our round-up or in the office after the round up.
Additionally, St. Mary’s Mobile Health Clinic will be available on May 8th at the Lucerne Valley Elementary School for immunizations and kindergarten and sports physicals. They will see anyone with straight medical, IEHP or no insurance. Please contact the school office for more information.
